• Replacement card due to a name change (original certification card must be submitted for this option): $10.00

Your request will be reviewed and certification status verified by All Care staff. We will notify you of your request status in 1 - 2 business days. Once notified, you may pick up your card in person, or we will mail the card at that time, depending on which method you've selected (on the form below).
